First Visit

First visits should arrive 15 minutes prior to treatment to allow time for an initial health assessment.  You can print out the New Patient Health History form here or from our New Patient page.  Complete the forms and bring them with you to your appointment

We recommend that you leave jewelry at home and bring a contact case for your lenses.

A 24-hour cancellatoin notice is greatly appreciated.
